Those who do not have internet access or printing ability may receive a New Patient Registration & Medical History form in the mail before their appointment, upon request.  Otherwise, we ask patients to arrive 15 minutes early to their appointment time to allow for paper work. Testing – If you know that you need blood work done, please mention this when scheduling the appointment, and for diabetes or lipid testing be sure to fast overnight before the visit. It is all right to drink water, though. Please Note: it is usually preferable to do the blood work in the morning. Examinations – Your first office visit may take up to an hour and follow-up visits only 10 - 30 minutes depending upon the complexity of your condition. You will be asked questions about your medical history and current health problems. Please bring any medications, vitamins or herbs you are currently taking as well as a list of past medications, vitamins/herbs when you come to the office.

Scheduling appointments –
When you call for a new patient appointment you will be asked for a brief explanation of your health need(s).  We do consider patients health needs and medication status before scheduling new patients.
Patient Registration & Medical History Form can be printed and filled out in advance. If you will bring this form completed at the time of your appointment this will help us to serve you in a more efficient manner. There will be some additional financial forms that may need signing at the time of your appointment.

Closed Friday Telephone Consultations –By appointment. Non-patient phone consultations are subject to a consultation fee.
Patient phone consultations - Our nurse will be able to handle many health and prescription questions. If you need to talk with the doctor on the phone, a time will be arranged for him to call you back.
Emergencies – After office hours, the voice mail system will offer an option to leave a message for the doctor.  Dr. Hansen will be notified promptly and will return your call as soon as possible. Occasionally, the physician may be away and therefore unavailable in case of emergency you may need to be treated at a nearby hospital Emergency Room or urgent care facility.
Medications & Prescription Renewals – All prescriptions and authorizations for renewals should be requested during normal office hours. The nurse or doctor will contact your pharmacist during regular business hours. Please try to plan in advance for prescription refills especially before the weekend. If you need a refill please call your pharmacy first and ask them to send a refill request. Please allow 24 hours for refill requests.
Financial Policies / Insurance – We are a family practice and lifestyle clinic. We seek to provide the very best medical care and health education possible. In conjunction with our philosophy we have chosen not to become part of any HMO's and are not contracted with any insurance companies besides Medicare. We are considered out-of-network providers by most insurance policies.

This policy allows the doctor to: 1) be the one to determine the treatments needed in your specific case in conjunction with the patients desires -- and not the insurance company. 2) spend the time needed with each patient without having to meet quotas or accept more patients than can reasonably be cared for by this clinic. 3) refer and receive referrals according to your medical need – and not according to preferential lists.

Insurance Billing
We will bill Medicare and other regular insurance as a courtesy to our patients. Co-pays are due at the time of service.

Uninsured Patients:
Accordingly, we ask our uninsured patients to provide payment for services at the time of your appointment unless other arrangements have been made in advance with the business office. We seek to keep our rates as reasonable as possible and do provide a sliding scale for low-income families (upon application).
